Connectional Health through our partnerships with other community based service providers can do a complete evaluation of the client, provide therapy when required, and refer out to a whole host of other services, including, housing, Medicaid enrollment, medical evaluation, and wellness advocates to help you to achieve a plan for overall wellness.
